Hold down Shift while clicking "Refresh". Otherwise, clear your cache. On Firefox, Tools > Options > Privacy > Private Data. Change the settings to clear your cache. Then hit "Clear Now". And after you have that set, you can just hit Ctrl+Shift+Del, and it'll clear it whenever you want.
